Medical Form
Northminster must collect a medical form signed by a physician after March 1 for your child to start school in September. Signed forms are valid for two years.

Safe Boundaries Form
The Northminster Nursery School community adheres to the Northminster Presbyterian Church Safe Boundaries Policy. We ask that all parents review the policy, and new parents please fill out the reference, disclosure and consent form found at the end of the policy information.
